Date of restoration of pension is given on Page 15 or 16 of your pension book.

Forward an application to CMA (OP) for restoration of your full pension indicating the fol:

1. PA No, Name ,Rank,
2. Date of Commission
3. Date of SOS / retirement
4. Date of restoration
5. Name of the Bank from where you are drawing your pension
6. Attested CNIC Copy.

You can verify your date of restoration from the above table. Due to clerical error, date of restoration in your pension book may not be accurate.
